The word "spastic" is derived from the Greek word spasmos, meaning "convulsion." In a strictly medical context, it refers to increased muscle tone and involuntary contractions, a condition known as spasticity. This occurs when the parts of the nervous system that control voluntary movement are damaged, such as in cases of cerebral palsy, multiple sclerosis, or stroke. In these clinical settings, muscles become rigid and resistant to stretch, leading to characteristic jerky or "spastic" movements.

However, the word's history is also a cautionary tale of how medical language can turn into an insult. Throughout the 20th century, "spastic" shifted from a clinical descriptor to a schoolyard insult and a disability-related slur. This change is especially strong in British English, where the word is widely regarded as highly offensive. Using it for a person reduces them to a physical condition and reinforces harmful attitudes toward disability.

Because of this history, "spastic" is no longer a neutral word for people. You may still hear the related medical noun "spasticity" in a doctor's office, but the adjective itself is usually avoided in general conversation and in patient-friendly language.

In some places, people have also used the word informally to mean clumsy, wild, erratic, or overly excited. Even in those casual uses, it often sounds insensitive because of its history. Clearer and safer choices include "erratic," "uncoordinated," "clumsy," "jittery," or "frenetic," depending on the meaning. Understanding the weight of this word is part of using English with care and respect.
