Tuberculin is a purified protein made from Mycobacterium tuberculosis, the bacterium that causes tuberculosis (TB). It is best known for its use in the tuberculin skin test, often called the PPD test. If you have been exposed to TB at some point in your life, even without becoming sick, your body may recognize this substance. When a small amount is injected under the skin, the immune system may react by forming a small, hard bump. This does not necessarily mean you have active TB, but it does show that your body has reacted to TB-related material and that more testing may be needed. It remains an important tool for finding latent TB infection.
Originally developed by Robert Koch in 1890, tuberculin was first hoped to be a cure for tuberculosis, but it soon became clear that its main value was in diagnosis. The test can sometimes give a false positive because of exposure to other mycobacteria in the environment, but it is still a key part of TB screening around the world.
Tuberculin is a sterile, complex substance derived from cultures of Mycobacterium tuberculosis, the bacterium responsible for causing tuberculosis. It is not a vaccine or a treatment; rather, it is a diagnostic tool used to determine if a person's immune system has been sensitized to the TB bacteria. This exposure could be from a past or present infection, and a positive result doesn't automatically mean the person has an active, contagious disease.
The most common application of this substance is the Mantoux tuberculin skin test. A specific form known as Purified Protein Derivative (PPD) is injected into the top layers of the skin on the forearm. After 48 to 72 hours, a healthcare professional examines the site for an "induration"—a raised, hardened area. It is the diameter of this firm bump, rather than the surrounding redness, that determines whether a test is positive.
A positive test indicates that the immune system has recognized the tuberculin and mounted a delayed-type hypersensitivity reaction. This suggests the presence of either active TB or a latent TB infection, where the bacteria are dormant but still present in the body. To distinguish between the two, clinicians use further diagnostic tools such as chest X-rays or blood tests (IGRAs).
It is important to note that tuberculin testing isn't foolproof. Factors like a prior BCG vaccination (a TB vaccine used in many countries) or exposure to non-tuberculosis environmental mycobacteria can trigger a false positive. Despite these limitations, the development of tuberculin was a watershed moment in medical history. Once a failed attempt at a cure, it revolutionized how we track and manage TB, allowing for the identification of at-risk individuals and helping to prevent the spread of one of the world's most persistent infectious diseases.
