Transferrin is a glycoprotein—a specialized protein with sugar molecules attached—found primarily in the blood plasma and tissue fluids of vertebrates. Its main job is to bind to iron and transport it throughout the body. Because iron is potentially toxic if left "free" in the bloodstream, it requires a chaperone. Transferrin serves as that chaperone, safely binding to iron ions to prevent them from causing oxidative stress while ensuring that cells receive a steady, necessary supply.

The protein picks up iron primarily from two places: intestinal absorption (the iron you consume) and macrophages, which are immune cells that recycle iron from old red blood cells. Once bound, transferrin travels through the blood until it meets specific receptors on cell surfaces that "unlock" the iron for use.

Clinically, transferrin levels and saturation are vital diagnostic tools. Low transferrin levels can indicate malnutrition, liver disease, or chronic inflammation, whereas high saturation levels may suggest hemochromatosis, a condition of iron overload. Beyond its transport duties, researchers study transferrin as a potential vehicle for targeted drug delivery, using its natural binding pathways to steer medications toward specific cells. It even plays a role in "nutritional immunity" by sequestering iron away from invading bacteria.
