trachoma

Trachoma is a chronic infectious disease of the eye caused by the bacterium Chlamydia trachomatis. It is not transmitted through casual glances; rather, it spreads through direct contact with infected eye, nose, or throat secretions. A major vector for the disease is the common fly, which picks up the bacteria from infected individuals and deposits them onto the eyes of others. Consequently, the disease thrives in communities with limited access to clean water and inadequate sanitation.

The clinical progression begins as mild conjunctivitis, which is inflammation of the membrane lining the eyelids. Over years of repeated reinfection, tiny follicles form under the eyelids and can eventually lead to severe scarring. This scarring may cause trichiasis, a condition in which the eyelashes turn inward and rub against the cornea, the clear front surface of the eye. Over time, this constant friction can cloud the cornea and cause permanent blindness.

Historically, trachoma was a leading cause of infectious blindness globally. Today, the international community fights it using the World Health Organization's SAFE strategy: Surgery to treat the blinding stage (trichiasis), Antibiotics to clear the infection, Facial cleanliness to prevent spread, and Environmental improvement (such as access to water and latrines) to reduce transmission.

The name "trachoma" comes from the Greek word trachys, meaning "rough," which refers to the rough texture seen in the scarred inner eyelid. It is a clear example of how poor sanitation and limited healthcare can turn a treatable infection into a life-changing condition. Current global health efforts aim to eliminate trachoma as a public health problem through mass drug administration and long-term improvements in hygiene and living conditions.
