Imagine your blood as a river flowing through your body. Normally, it's a smooth, steady current carrying oxygen to all your organs. Now imagine that river getting crowded with too many red blood cells. That is essentially what happens in polycythemia. It is a condition in which there are too many red blood cells in the blood, which can make the blood thicker than it should be. This can slow circulation and raise the risk of clots.
It is not something you catch. It may happen because of a bone marrow disease (*polycythemia vera) or because of another problem, such as lung disease or living at high altitude (secondary polycythemia*). Polycythemia (pronounced pol-ee-si-THEE-mee-uh) refers to a condition characterized by an abnormally high concentration of red blood cells in the bloodstream. Think of it as having too many passengers crammed onto a bus—it makes everything more crowded and difficult to move efficiently. This thickening of the blood can lead to various complications, including an increased risk of blood clots, stroke, heart attack, and organ damage.
There are two main types people usually talk about. *Polycythemia vera is a rare bone marrow disorder in which the body produces too many red blood cells regardless of oxygen levels. It is often linked to a genetic mutation. Secondary polycythemia, on the other hand, develops because of another condition that causes the body to produce more erythropoietin, a hormone that stimulates red blood cell production. These conditions can include chronic lung disease, certain heart problems, kidney tumors, sleep apnea, smoking, or even living at high altitudes where oxygen levels are lower. There is also a less common relative polycythemia*, in which the red blood cell level appears high because the liquid part of the blood is reduced, such as during dehydration.
Symptoms can be subtle initially and might include fatigue, headache, dizziness, shortness of breath, blurred vision, and skin redness (especially on the face). Diagnosis involves a complete blood count (CBC) to measure red blood cell volume and other tests to determine the underlying cause.
Treatment varies depending on the type and severity but often includes phlebotomy (regularly removing blood), medications to suppress bone marrow activity, and managing any underlying conditions. It's important to note that polycythemia is a serious condition requiring medical attention; it's not something to self-diagnose or treat.
