A fistula is an abnormal connection or passageway between two body parts that should not normally be connected. It may link two organs, an artery and a vein, or an organ to the skin. It is not a form of cancer. Instead, it is like an unintended detour inside the body, allowing fluids or substances to flow where they are not supposed to go. The word comes from the Latin fistula, meaning "little pipe."

Fistulas can occur in many parts of the body. An arteriovenous fistula connects an artery and a vein, while a gastrocolic fistula links the stomach and the colon. They often develop as a result of injury, infection, inflammation (such as from Crohn's disease), or surgery. Occasionally, they are congenital, meaning a person is born with the condition.

One particularly heartbreaking type is the obstetric fistula, which develops during obstructed labor when the blood supply to the tissues of the birth canal is cut off. This can create a hole between the vagina and the bladder (vesicovaginal fistula) or the rectum (rectovaginal fistula), leading to constant incontinence—the uncontrolled leakage of urine or feces. These fistulas are tragically common in areas with limited access to quality maternal healthcare, causing immense physical and social suffering for women.

Diagnosis usually involves imaging tests such as CT scans or MRIs, sometimes combined with a special dye injected into the suspected area to trace the path of the leak. Treatment depends on the location and severity of the fistula but often requires surgery to close the abnormal connection. While the condition is challenging, successful treatment can dramatically improve a person's quality of life.
